Most students think Step 1 preparation begins the day they open a question bank. It doesn’t. It begins in the first week of basic science, the first time a lecture goes by faster than you can write it down, and you decide whether to chase it or let it go.

That’s why we don’t treat Step 1 as a separate project that happens at the end of year two. At Windsor University School of Medicine (WUSOM), preparation is built into the MD program and carried through Basic Science, semesters 1 to 5 of the 10-semester curriculum.

The faculty who teach you anatomy, biochemistry, and pathology are the same people helping you get ready for the exam. Here is what that looks like in practice.

What is USMLE Step 1?

Step 1 is a multiple-choice exam taken at a test center. It tests whether you understand the science underneath medicine well enough to use it on a patient: the mechanisms of health and disease and how you apply them to a clinical problem.

As of May 14, 2026, the exam has an updated testing interface. The content, scoring, and pass/fail standard have not changed. What has changed is the structure. You now sit fourteen 30-minute blocks of up to 20 questions each, instead of seven 60-minute blocks. It is still 280 questions in one day, with 55 minutes of total break time and an optional five-minute tutorial.

Step 1 has been pass/fail since exams taken on or after January 26, 2022. That took some pressure off, but it did not make the exam optional. You still have to pass it to keep moving forward, and residency programs now look harder at your Step 2 CK performance, clerkship evaluations, letters of recommendation, and research. A pass is the starting line for all of that, so it still deserves real preparation.

USMLE Step 1 Preparation: Why It Starts Early

USMLE step 1 requires all the knowledge you gain in the first two years. Class performance is one of the most reliable indicators of readiness, so be mindful of your own and tackle any weak areas sooner than later, well before the test day. If you realize that you are missing something in semester 2, it’s no big deal; you can fix it easily.

However, the same gap in your last month of dedicated study is a crisis.

It is so much easier to have a set time to study when students have already prepared for Step 1. The foundation is already in place, so those final weeks are not about learning it for the first time but polishing. The typical suggestion is about six to eight weeks of full-time study, including breaks, and that plan only works if you don’t start from scratch.

The Role of WUSOM’s Basic Science Faculty in Step 1 Success

The Basic Science Faculty at WUSOM plays a pivotal part in Step 1 Success. We believe the greatest untapped asset in medical school is faculty, so we want that to be different. The key to success on the USMLE Step 1 is to master biomedical concepts in a meaningful and integrated way, not just memorize them.

WUSOM expert faculty members in the various areas, such as the Department of Molecular Sciences, Pathophysiology, Anatomical Sciences, and Pre-Clinical Sciences, guide students and engage them in meeting this objective.

Students who bring their questions to faculty regularly tend to get more out of them, and these are the situations where it matters most.

What WUSOM Faculty Actually Do

  • Delivering Individualized Academic Support: All faculty members are consistently available to support students individually to ensure that they master key concepts and progress through their courses.
  • Leveraging Expert Insights: Faculty members have years of experience and know what sorts of topics and core concepts will be heavily emphasized on board exams, allowing students to focus their study in an efficient manner.
  • Integrating Disciplines: Faculty plan the curriculum around 13 modules that are systemically connected, incorporating anatomy, physiology, biochemistry, pathology, pharmacology, and microbiology so students can learn and understand how these concepts are applied to one another in Step 1 tests.
  • Partnering with Student Support Services: In collaboration with the Student Advisory and Support Program (SASP), faculty help students recognize gaps in their knowledge early on, develop effective test-taking skills, and create a plan for studying USMLE Step 1.

Key Strategies for USMLE Step 1 Preparation at WUSOM

Studying for an 8-hour comprehensive board exam requires a focused, evidence-based approach. There are a number of successful strategies that WUSOM recommends to help students prepare for the amount of material needed for Step 1.

  • Start Reviewing Early:

Step 1 is a two-year course in foundational sciences, so it is imperative to develop good study patterns from the first day of medical school. The repetition of the core concepts helps these get stored in the long-term memory.

  • Utilize Practice Tests and NBME Assessments:

Practice taking National Board of Medical Examiners (NBME) self-assessments helps you get used to the exam format and identify areas of weakness.

  • Master Central Concepts and Classic Presentations:

Focus your study efforts on core concepts across major disciplines and prioritize the key characteristics and presentations of diseases rather than rare exceptions.

  1. Break Down Practice Questions:

Familiarize yourself with how USMLE questions are structured so you can quickly analyze data, eliminate incorrect options, and apply basic scientific principles to clinical scenarios.

  1. Maintain Balance and Healthy Habits:

Long study sessions do not equate to improved learning. Regular breaks, physical activity, proper nutrition (brain foods), and a regular sleep pattern are important for optimal cognitive functioning and avoiding burnout.

Your Next Step

If you are considering medical schools, inquire about what they do to prepare you for Step 1 and when. At WUSOM, that preparation begins at the start of the MD course with faculty and academic support services to help you get through it.

Frequently Asked Questions

Q: What is USMLE Step 1?
A: It is the first exam in the three-part USMLE sequence, followed by Step 2 CK and Step 3. It tests your understanding of the foundational science behind health, disease, and treatment, and your ability to apply it to clinical problems.

Q: When do students take Step 1?

A: Usually at the end of the second year, after completing basic science. It’s recommended to register about six months before your preferred test date so you can get the date and location you want.

Q: How long is the exam, and what is the format?
A: The exam is multiple choice and takes about eight hours in one day. It has fourteen 30-minute blocks of up to 20 questions each, for 280 questions in total, plus 55 minutes of break time.

Q: Is Step 1 scored, or Pass/Fail?
A: Pass/Fail. The change applies to exams taken on or after January 26, 2022. Residency programs now tend to put more weight on Step 2 CK, clerkship evaluations, letters of recommendation, research, and overall academic performance.

Q: How long should I study during a dedicated period?
A: A common recommendation is about six to eight weeks of full-time study before your test date, depending on how comfortable you are with the material. Starting to review early in medical school makes this period much more manageable.

Q: How does WUSOM help students prepare?
A: USMLE preparation is built into the MD program and emphasized throughout Basic Science (semesters 1 to 5). Faculty provide individualized support, and Student Support Services help students develop study techniques and strategies for medical school exams and the USMLE.
