• Complete Application for a Student Visa
• Passport
• Birth Certificate (Original)
• Immunization Card (Must have three doses of Hepatitis B, two doses of MMR, two doses of Polio)
• Bank statement (Totaling $12,000 USD or higher (personal or from parents/guardians) or Loan bank statement from sponsor accompanied by a notarized sponsorship letter
• Police Record (Must not be older than 6 months)
• VDRL/RPR (Must not be older than 6 months)
• Mantoux for Tuberculosis/PPD (Must not be older than 6 months)
• School Acceptance letter
• Return Ticket
